Creatine and the brain

The best evidenced muscle supplement in existence has a brain story too. It is real, smaller than the hype, and strongest when the system is stressed.

In short

Creatine buffers cellular energy, and the brain is an expensive organ, so the rationale for creatine and cognition is real. The evidence is smaller than the current enthusiasm: meta-analyses find modest memory improvements, driven mostly by older adults, while the largest trial in healthy rested adults found nothing. The clearest signals appear when the system is stressed: sleep deprivation, vegetarian diets, ageing. EFSA assessed the cognitive claim in 2024 and declined it. Creatine is having its cognitive moment. The honest version of that story is worth more than the hyped one, and it points somewhere specific.

01What creatine is

Creatine is an energy buffer: stored as phosphocreatine, it regenerates ATP in the seconds when demand outruns supply. Muscle is where supplementation raises stores most, which is why the muscle evidence towers over everything else, and the brain holds its own creatine pool, turned over more slowly and raised less easily by supplementation. The rationale for a cognitive effect is genuine; the question is what the trials found.

02The muscle record, briefly

For completeness: creatine monohydrate at 3 to 5 g daily is the most effective legal performance supplement known, with three decades of safety data, per the International Society of Sports Nutrition's position stand.1 In the EU it holds an authorised claim for physical performance in successive bursts of short term, high intensity exercise, at 3 g a day.2 None of that is in dispute, and none of it is about the brain.

03The brain evidence, honestly

The cognitive record is genuinely mixed. A systematic review of six randomised trials in healthy adults found possible improvements in short term memory and reasoning, with other domains inconsistent.3 A 2024 meta-analysis of 16 trials found a small memory improvement and little else, with the clearest effects in older and clinical groups.4 Against that, the largest single trial in healthy, rested adults, 123 people at 5 g a day for six weeks, found no significant cognitive benefit at all.5 A small memory effect that concentrates in older adults, and disappears in young rested ones, is not a nootropic revolution. It is a repletion story.

04Strongest when stressed

The pattern sharpens at the edges. Under 24 hours of sleep deprivation, creatine preserved cognitive performance in a small trial.6 In vegetarians, whose dietary creatine intake is near zero, six weeks of supplementation improved working memory and reasoning.7 Low stores, high demand, measurable benefit; full stores, no demand, nothing to find. That is exactly how an energy buffer should behave, and it is the honest frame for the whole literature.

05What EFSA just said

In November 2024, EFSA assessed a proposed claim for creatine and cognitive function and concluded the cause and effect relationship was not established.2 That does not mean the research is worthless; it means the regulator read the same mixed record this page just described and declined to let it become a marketing promise. A Library that cites EFSA when it cuts against botanicals owes the same citation when it cuts against the internet's current favourite.

06Why it is not in this system

The honest reasoning

Creatine is a good supplement doing a different job. It works at gram scale, 3 to 5 g of powder daily, which does not belong inside a capsule architecture built on milligram doses. Its authorised claim and its strongest evidence are physical, and this system's lane is the day's structure: focus, resilience, recovery. If you train, creatine on its own, as monohydrate, is one of the few supplements this Library would call obviously worth its money, alongside this system, not inside it. That is the whole answer.
